Dean’s Certifications or Disciplinary/Academic History Requests

Many institutions and programs, including graduate and professional schools, will require the applicant to provide information about prior disciplinary or academic history, whether or not a Dean’s Certification is required. It is advised that you answer such questions truthfully and disclose information pertaining to your records. Failure to disclose this information can result in severe consequences in the admissions or employment process and, for law school applicants, later in the bar admission process. If any academic or disciplinary action is taken against you after submitting your applications, you must update the schools or program to which you have applied.

Dean's Certification forms are required by a small number of graduate and professional schools, as well as on some transfer applications. Many schools also require a Dean's Letter or Certification. Certifications are required either before, or after you have been accepted. Read the application carefully for the institution or organizations policy or call the receiving admission's office if you need clarification.

Your record of academic probation can be found on your unofficial Tufts transcript (not your official transcript). You may still have access to it online. Your official transcript will never include a record of academic probation.

Academic Standing reflects a student’s progress toward degree completion, including credits earned and grade point average. Academic Probation for a Tufts undergraduate student indicates a concern about grades and credits earned; it is not a reflection of a disciplinary concern. Disciplinary Probation for a Tufts student at any level indicates disciplinary action having been applied as a result of a responsible finding through the Student Conduct Resolution Procedure (SCRP).

Completing Certifications at Tufts

  1. If you will be applying or transferring to any jobs, graduate or professional schools, or other programs that need a copy of the academic or disciplinary records from the time of attendance at Tufts University, you must fill out a Disciplinary/Academic History Request Form.
  2. If the jobs, graduate or professional schools, or other programs have supplied you with documents for us to fill out, please attach it at the bottom of the Disciplinary/Academic History Request Form.
  3. Allow a processing time of at least 2-12 business days before the application deadline.
  4. The letters and/or forms will be sent directly to the requested third parties.
